You need to arrange those 3 medallions correctly. Refer to Jeremy's book.
If I remember correctly it should be:

Are you rotating the pieces too?

You can find the shape of that puzzle in one of the books and don't forget that you can rotate these items.

There are multiple video walkthroughs online, so I'd suggest checking one of those for the exact rotation of the medallions needed. If it still doesn't work for you, even though you place and rotate the pieces correctly, then please report it as described in the following thread, with your save game attached:

My medallions just don't even show up and I have both of them. Only the eagle pops up what a garbage game.

Goes by and gets medallions both of them and can't do puzzle JUST GREAT asshats.

Sorry to hear that you've encountered a bug with the medallion puzzle, but no reason to be rude.

This does not seem to be a very common issue, which means it will be tricky to pinpoint without affected save files, so reporting it as instructed in the following thread, with your zipped "Saved" folder attached, is the best way to get the developers to investigate and hopefully fix it:

I have the same issue, I had both medallions, omen and blessing, now the blessing is missing and I can't open the second door

Reiterating once again that it will be incredibly helpful for the developers to have save files from affected users, so please, if you have not done so already, log a bug as instructed in the following thread:

In the meantime, I have made back-up saves at the start of each level for both Edward and Emily. You can find them in the following Google Drive folder, under sub folders "First playthrough as Edward" and "Second playthrough as Emily". Simply download the respective "2-4_LafayetteCemetery" save_10.sav into your save folder and rename it "save_10.sav" to restore progress to the beginning of the cemetery chapter. Full instructions and more information can be found in the READ ME FIRST file in the Google Drive folder:
