Just does it on its own. Means your attack has a chance to strike multiple times. Higher the chance, higher the proc rate.

I think it just means "That" hit, so it would proc more from multiple strikes, but be more beneficial on a 2 handed weapon. The great axe with multistrike during surge is just nutty, especially with the item that makes you always frenzied with grey health.
