EDIT: in hindsight, a little snippet wouldn't do anything wrong, I think?
Some are obvious, but why not something like
ENEMY REVIVE
- Some enemies have a second chance! -
TRAFFIC LIGHT
- Green light go, red light stop! -
VOIDLING
- The Void has touched some of the creatures here... -

Also, completing any failed challenge rooms (which you have to in order to continue playing) will damage you down to a fraction of one health bar. These two things happening back to back have made me quit out more than once. Like, f*** you too, game, damn.
This is an easy fix by just setting the room parameters for that challenge not to start in tiny narrow passages. Like spawn in the middle, or make sure that challenge has a larger opening at the entrance.
I brought this up in the discord with a screenshot of my own showing the problem and was told indirectly that the devs had mentioned being aware of and intending to fix that specific issue (tight hallways)
