For Manor, it can be in safe houses or landmarks, and there are some landmarks that players often overlook, which can also have objective portraits spawn. Some examples would be a half-room situated in a somewhat open section of the maze with a fireplace and two chairs in front of it (it can spawn on a chair), the landmark with the status of a woman in the center of a 4-way intersection (it'll spawn in front of the statue), the hallway with a high ceiling (I call it the Tallway; it can spawn on the carpet), and a final example I can think of is an open area with a bookcase in the middle of it (it'll spawn on a book on the ground at the base of the bookcase).
Is Sewers one of the exceptions, or perhaps rares don't have to be in POIs? We did a rare case and I apparently found the cosmetic but walked past it cause I didn't see anything special, and we didn't return there because I was told they can only spawn in safehouses or at POIs. It was just in a dead end near (directly behind) one of the safehouses with one of the non-usable pipes/valves.
Sewer can have cosmetics spawn at safe houses and landmarks, they're just so much more likely to spawn in safe houses depending on the size of the maze. This is because Sewer objectives are a little special in the sense that the colored valve landmarks cannot have cosmetics spawn in them. Because the objective landmarks take up so much space, if you frequently do smaller Sewers, you won't see regular, non-valve landmarks too often because of how much space they take up. On the off chance you do have a regular landmark though, there is a chance the cosmetic will spawn there instead of one of the safe houses.
Cosmetics can't spawn in dead ends though, so the star on your map was likely just off-center a bit and intended to mark the safe house. This can happen sometimes depending on how maze generation goes during the loading screen; there can be slight adjustments that may throw off markers on the end-game map just a tad.
