In the exploration age, i successfully opened the Nepal Civ. Yet when I transitioned to the modern era Nepal was locked out. I had my city's with the needed amount of mountain tiles in them. I was playing as Trung Trac as the Abbasid empire. Yet this should not have happened. There is a bug there that needs to be fixed.

Can confirm this happened to me as well (playing as Bolivar and Bulgaria at the time).

Same issue. Playing through two games as Pachacuti because...mountains. Finished Exploration as Mongolia with plenty of mountains in my cities and Nepal listed as unlocked on the Exploration Unlocks screen. But when transitioning to Modern, Nepal is then listed as locked. Experienced the same issues with Great Britain in the last few weeks- which I unlocked dozens of times since I usually build more than 2 fleet commanders.

Seriously how did they mess this up for a second time in a row?
