I'm pretty sure the model/lore comes with both options, men and women? Could be wrong but I feel like I remember that.

Probably some kind of censorship decision forced on the devs. If you look at the Penitent Engine in the old DoW Soulstorm game, it has a hot woman on it.

The models in the game reflect the plastic kits, no censorship required.

Penitent engines can have any form of sinner in it similar to arco flagellents. It's the mortifiers that are usually reserved for Sororitas who fail their tests of faith, or for those who REALLY fail they get the anchorite treatment. Though the anchorite can also be a reward for the most faithful sisters who were mortally wounded, acting like a full Sororitas dreadnought. The main difference between the two anchorites is that while both versions are designed to keep the owner from dying. The sinner is kept in constant suffering while being kept alive so that they will have a very hard time being freed from the pain while honored anchorites are kept alive but without the machines inside that keeps them in a state of hellish pain.
