How is the game monitized?
The free to play tag is very concerning

Zero monetization whatsoever. Literally just free. A sweet, short, honest game that just wants you to have fun.

The creator had a craving for sharply dressed demon girls so he made the perfect game to attract a lot of artists to draw fanart. Genius.

You don't even have to pay for the artbook. If I remember correctly completing the game or 100% it (which is easy) will give you a link to it for free.

^artbook code is free at the end of the secret ending. The dlc artbook is just a way to support the dev

I agree that free-to-play is a red flag. Most of the time, free-to-play really means pay-to-win or pay-to-avoid-grinding. However, there some exceptions. Some games really are free. Helltaker really is a free game.

On the other hand, there is Helltaker merchandise available for sale, so the developer is not completely altruistic. Perhaps you could consider the game to be an advertisement for the merchandise.

"Free" and "Free to Play" are two separate things on Steam, there's a difference and the community tags aren't always accurate anyway.

there is a $10 (USD) DLC
it only contains content that is available in the artbook that you can download for free from the dev's itch page

You say it like free to play games are a bad thing. Nothing wrong if the creator didn't want to put a full-price on this game(considering how ridiculous most of the games have put it, and it feels like they didn't deserve the full-price), so its a blessing that the creator made this game for free!
