I succeeded at the achievement
Multiple crypts OP (have 3 now) and I can say that placing multiple mazes of memory does nothing (for as far as I notice)

Multiple ancetral cryps seem to have weird effects on health
With three I had 19K health, placed a fourth and now I have 8K...
I find it highy unlikely to be so lucky as to get it before the boss.
You need an enemy with a soul to die next to a battle field
20% become ghosts
20% of that become ghost of a ghost
20% of that (though it feels lower) become primal matter
33% chace it drops a golden card
25% chance that golden card is the maze of memories
For a total of 20%*20%*0.2*33%*25% = 0.066% chance to get a maze of memories per enemy with a soul fought (about 1/1500)
It does seem like having multiple wasn't intended and IIRC they patched it.
