Rat Ogre grabbed an enemy and killed it, and it resulted in my hardcore run being lost

I don't know what exactly happened or why, but rat ogre from Losomn grabbed an enemy as part of its normal interactions with the local population. It killed it, and it scripted it as being my death ? I was still alive and walking though, and by all accounts, I should not have died.

Thankfully this was early in the run, but it's still frustrating to see.
