Game Breaking Tal'Ratha Door Bug

Currently on apocalypse mode N'eurd (my final world) before going to the Root Earth with the boss variation being Tal Ratha. Found the Soul Spark, went to Tal Rathas door to initiate the exchange - all went well until after finishing his dialog. I tried to interact with the door to give the quest item but it didn't work. I could spam the interaction button but the item prompt menu would not pop up. I’ve tried fast travelling, resetting the game, verifying game files; Nothing seems to work.

Please fix this horrendous bug......

there two "entrances" to the Tal Ratha chamber - one with communication device, another just a door. You need to enter Forgotten Prison from Eon Vault to get the boss fight. Just in case - are you sure that you are entered the right part of the location?

Thanks for the comment; Yes I'm certain I'm at the right area, I've done this numerous times on different difficulties already.
