Instant death after leaving boss room in boss rush.

My friend and I were doing some boss rushes this morning and we've failed the run now twice because after killing the boss and running towards the boss chest we've hit an instant kill trigger right after reaching the threshold for the boss area.

Has anyone else encountered this? We've done dozens of runs throughout the last few weeks and have never had this happen before but now it's happened twice in the span of an hour.
