Bug Report - Ring of Omens Missing

I was able to re-roll adventure mode until I got Cathedral of Omens, and completed the puzzles to open the door with the additional sliding panel leading to some essence and a ring. After completing the puzzle the door opened just fine, and I took the ladder down, but there were no essences or ring to loot.

If I understand this puzzle correctly, that blood door shouldn't have even opened unless it was a Blood Moon, correct? I've vigorously checked my character's inventory, as I've been looking forward to this ring, and it is nowhere to be found. Even if I had somehow found it before, I should've been able to pick up a duplicate for some scrap.

Have seen other reports about issues getting this ring in adventure mode, but almost every other report I saw simply said the door wouldn't open, not that the ring wasn't there.


TL,DR: Did the puzzle, blood moon door opened, no ring available.

I did it last night and the floor door didn't open for me to even look with a blood moon. I heard that you have to have 15 blood essences in your inventory which I had 14 but who knows what is going on. What got me I was in adventure mode so I waited about 3 hours in order for the blood moon to show up and I didn't get it away.

I replicated the exact same steps taken the first time, and the ring of omens was there the second time. I wonder if something happened that prevented the rewards from the puzzles loading the first time around? I also noticed that when I completed the first puzzle, I didn't receive the seed either. Glad to have the ring at least, but tedious to have rerolled and reloaded the area for another couple hours to get this.

There has been a problem since launch where static loot can spawn outside of the map. I've personally experienced this with Plasma Cutter (twice) and more recently the Cost of Betrayal DLC item.
Others have reported similar issues with various puzzles. The only solution I know of is re-rolling and hoping it spawns correctly.

Same happened to me today. Finally got blood moon, right door opened, floor opened, but only the 3 blood essence were there.... no ring! :(
