Also keep in mind that you can hand in only 1 per run, so if you don't hand it in after you found one, and roll a new Adventure Mode, the item will disappear from your inventory.

Apologies, mate. Was just trying to help. ^^'' Just tried to Google for this bug, but nothing popped up yet. Must be something new or recent, maybe they did some hidden patching and messed up something. Does this happen in coop or solo or both? This is just a wild guess / theory, but maybe it's connected to the "Thank You For Being a Friend" achievement. Since Walt is giving you all the role knowledge which you will relay to Dwell, but haven't found any proof about it being a prerequisite for this one.
Just read some posts under a few achievement sites that it is indeed a bug and / or it can randomly get bugged. Hopefully the devs will patch it out soon.
Dwell initially gave me the dialogue properly on my first One Shot Adventure Mode run. I have now re-rolled my Campaign AND Adventure Mode 3 times each and fetched the Trinity Memento and Dwell does not notice in all six attempts.
