How do the maps work?
Played a bit at launch. Got busy. Finally come back to it and started a new game.
First thing i notice is the map i start on is different (after the intro bit).

I have the DLC so i'm assuming this map is that (its sort of got a Bloodborne vibe which i'm loving)

But are the maps randomised at all? Is any of it procedural or are they all pre-made maps and just the order of the rooms or worlds is random?

Campaigns and adventures are mostly random with the exception of the special "One Shot" version of each DLC's story.

Each main world has 2 or 3 different stories and each story has a few plot related locations that always show up. What you travel through to get to the next required location, as well as optional areas, are completely random. Things that always show up in a given story tend to have a set layout.

The layouts of some areas and dungeons have varying levels of procedural generation in play too. Some of the more unique dungeons have a set layout, while others are largely random.

A set of generated tiles will fill in randomly within each of the 3 world.

When starting a campaign, you start with one of the three main worlds chosen randomly. You get the other two later in the campaign.
In a world, you get one of the two storylines (three with the DLC for that world) chosen randomly.
For a storyline, you get a few areas that are not random (and unique to that storyline). The world boss you'll encounter is also linked to the storyline you got.
Other areas are random.
Within an area, you can also get an event. There are specific locations in an area where an event can appear, but it won't always appear and sometimes an event appearing prevents another event from appearing. Some events can only appear within a specific area, some can appear in a few areas of the same world (but they can't appear several times in the same iteration of a world).
Also, within an area, there are some items to found. Some are always the same at the same place, but there are also a few place where the item appearing is random.

prefab tiles are used to generate a world, they have specific entity nodes for items and mobs which can randomly appear at those nodes dungeons however can be pretty static outside a few "left/right" tiles that allow for an event room to spawn. Some dungeons are entirely static, but again the entity nodes can be a bit random.

In procgen terminology it's called "vaults". Puzzles, story rooms, boss rooms and some other major setpieces are always the same, they're glued together by random corridors and rooms. (And dungeons usually have one guaranteed setpiece which you can recognize on its name + maybe one or two random bonus ones)
