Ravenous Medallion did not spawn

Has anyone else ever gotten to the kitchen of the Great Hall and found there's no Ravenous Medallion on the counter? I've done this event a few times, and this time I rerolled Losomn on Adventure mode just to try and get the Rune Pistol, but the thing isn't even here.

Are there any conditions that have to be met before the medallion appears? Does it despawn if you picked up the Soulkey Tribute already? This is pretty frustrating because it took me a while to get this particular Losomn layout, and now it looks like that was time wasted anyway.

Up the stairs, to the left behind the random area? I've never had it not spawn on the map before. Didn't even know that can happen.
