Titan's Reach + Navigator's Helm

Things like this should not happen at all.
First I had to go through 2 Adventures to get the Helm, so I finally could open up the Door in Titan's Reach (Navigator's Helm is needed).
Went back to campaign mode, where I knew I had Titan's Reach.
Door opens and TADA!!! You get ♥♥♥♥ ALL!

The weapon which is supposed to be there, was not there, my character did the "Oh nice" bit, when you find a weapon.
Ok look it up people say "Bug just reroll adventure."
So I did.
I opened the door a Second time, and my reward? 100 Scrap.
Again no weapon, nothing.

Redoing N'erude over and over and over again, to try and get the lucky 17 of RNG for it to not Glitch out, bug out, or break, while also having to pray to some cosmic horror for the rng not to say "doesn't spawn this time." is really bad.
Having secrets to find, good, having puzzles to solve fun in most cases.
Having a not guaranteed spawn of the items, gears or weapons, bad.
Also not saying what the weapon is, because it could spoil the surprise for people who doesn't know.
