How to get to Titan's Reach?

I didn't see it in my main campaign - do I need to reroll adventure mode to get to this or does it always appear and I may have missed it as a side dungeon or something?

its just a random spawn on one of two open zones while you play Nerud, and yeah if you dont get it in your campain theres a high chance that you can easily get it in your friends world or just roll it in adventure mode

Can I get Titans Reach as the first entry point after rerolling Adventure Mode? I've re rolled a few times and I get either Seeker's Rest or Forgotten Prison as starting. I had seeker's rest in my campaign so I chose Forgotten Prison in the Adventure Mode but it led me to Abyssal Rift which was also the second zone in my Campaign. Im not sure if spending all this time will lead me to Titan's Reach even after this re-roll
