Can't Find Putrid Domain
I got hatchery instead. Is it possible to not get dungeon because some other dungeon spawned in its place?

You can open a world on adventure mode for specific levels to get the other dungeon.

When I reroll adventure mode I always get Yaesha.

Yes dungeons rolls are rng and you have to complete the world before you can reroll it in adventure mode. So for you it seems you'll have to finish Ne'rud before trying to get Putrid Domain in adventure mode

Yes. There are more Tilesets per World than there are that actually spawn per Campaign. You will not experience every location in 1 run through

And yeah, its quite possible to not get a dungeon in a 'run'. Actually there are places you definitely won't see in just one run through a zone.

Have to beat a zone at least once before you can make one in adventure mode for you to run through.
