This did not drop for me in the Labyrinth and when a friend picked it up in their game with me as a co-op player I did not receive it - what gives?

i think it's a guaranted drop, sounds like a bug.
you will have to redo the campaign wich is not so bad, probably 1 hour to beat first map anyway
