Missing easy(?) perks?

I've seen some posts about the harder to get perks (reflect healing fireballs, coop, etc.). I've been playing some 20 odd hours and have never seen some of the easy ones. Are they locked by a certain weapon or perk combo that I might not have tried? (perks 4 and 8 in the first row)

Did you ever figure out how to trigger the last perk on the first row?

Did you ever figure out how to trigger the last perk on the first row?

I did finally!

I forget if there was some secret to it, or if I just needed to get lucky (I think it was new game+ gear related). It definitely took longer than expected, as you might be able to tell from my 30 hours of playtime.
