According to the answer. The main one who's coughing is the Johnny himself. His mouth may not overlap, ofc, but all of the character's mouth (Except for Baldi sometimes) don't move accordingly on their line.
- Does playtime's mouth move when she says "Let's play!"? No
- Does Principal's mouth move when he says "No bullying in the halls!" No
- Does Mrs. Pomp's mouth moves when she says "Just reminding you to be in my class in 2 minutes." No
Because of theses proofs we can say that the main one who's coughing is Johnny. Also, Johnny is exclusively the only character who has lip-sync animations for everything he says.
Also also, the coughing sounds masculine to me, while Johnny was voiced by Mystman's sister. This, however, could just be a stylistic choice since Johnny is a male, so I don't know if this point really stands.
So I guess it must be the player the one coughing, or maybe an easter egg, who knows since the game is still being developed.
It's just insane how far we came after a question
