Do the accept ending and get that, do the refuse ending and get that, then get the eyes and do the refuse+extra ending.
If you've already got both eyes, you can only get the two endings.
This is such a bad design choice. There is no indication of a "point of no return" with opening some random chests and receiving the eye balls. It also feels incredibly silly since it is an impossible feat for the player to just drop the eyeballs. You are given a choice with the throne so why not give the player the choice with the eyeballs? From software games have more flexibility with the endings. In bloodborne you have to consume 3 items for the final ending. In Eldenring you are given choices in the end for all but one ending where you have to do a specific thing and even that is possible to reverse. Very disapointing choice from the devs.
There are 3 distinct endings, but if you're only worried about the achievements, there's no problem.
Once you actually beat the game (and get an ending) they become "calm" again and offer you the choice once more.
