Even if you track him the whole mission, he can simply walk towards hostages/bodies and you can't kill him. You have to let him cuff someone if he didn't walk into a secure area.
As a side note, I had him walk into the VIP room in Dirty Ice consistently, the manager room and the security room in the basement, which worries me because that could mean he is scripted to track bodies.
Honestly the thing that really bothered me about him is how he caught me in the VIP room while the door was closed (no line of sight) but based on sound since I was running inside to bag stuff fast.
That means once a bag is taken, the only "safe" rooms for bags and civs is the bathroom and the room where you clean the jewellery (opened by the manager).

This is 100% the truth. The lead guard will always change paths once certain objectives have been completed during a heist. You have to be aware of this and plan by watching the Lead Guard. His pathing on Dirty Ice is actually very easy to learn. Also, stealth is generally better in this game when you have at least 2 people, as one person could just you cameras at all times to keep an eye on Lead Guard and other guards.
In black ice the lead guard starts patrolling inside shortly (1 minute?) after you disable the alarms.
In touch the sky the lead guard will start patrolling the main office and bedroom after you unlock them.
it's not a glitch, you're just bad at avoiding him whenever he pops up. if it was a "search" you'd get arrested on sight instead of being escorted out
Having beaten that level on stealth overkill.
You are correct, the second you touch the jewelry in the back room. The lead guard script kicks in. He will constantly patrol around that jewelry area. You can loot the loose loot, but the lead guard script kicks in exactly when you pack up the jewelry. Collect all the loose loot, unlock all the cases. Then with the micro cam watching the lead guard outside. Wait till he is far from the side door and throw all the jewelry bags into the special sorting machine.
I'd recommended using those micro cams.
Plant the two cams outside alleys to watch the lead guard and the other guard with the blue key-card.
Burn your two radios / pagers in getting the guard smoking next to the truck at the start of the round and then get the cam guy. Don't rush the cam guy though.
You need to focus all your time in getting into the basement before the manager leaves.
Slap a microcam or motion sensor directly on the Lead Guard. With the microcam, you should be able to access it, point the camera down or up, and then mark the guard its attached to so you keep him highlighted.
Do the other objectives, grab the keycards...
Use both of your pagers to kill both of the two guards that walk through the hallway containing the vault (the smoker and the guard in the lower alleyway). Highly recommended that you get the second skill on the Hacker tree so you can ignore the cameras watching the jewelry room and hallway. I hid my bodies behind the computer desk.
That means the only person who can bother you in that hallway is the Lead Guard (though keep watching the hallway leading to the lobby, there's a guy who walks into the break room occasionally). But you have him on notice, so he shouldn't be a problem. Grab everything in the room, then grab any civ and have them hit the button to open the vault.
No need for cameras or sensors but they do help if still don't know the exact path guards take
