1 hard crash to desktop
2 times where a new mission would go to loading screen and never load
a handful of times where using Ansel photo mode breaks AI NPC pathing

I have been playing on DX12 the whole time, i havnt bothered trying DX11 because the game has been working perfectly for me.
Literally the only thing i have come across is a civilian was stuck inside a bench lol.
there were also a ton of minor bugs, like one of the hippie chicks hair just straight changing color, being able to put the camera underground in Tunguska, and just so much more.
