About Kurt issue

Kurt always dies from heart attack and then falling from the rope in act IV when trying to take back the camera with Kasha. I tried to encourage him, to threaten cutting the rope, but the issue remains ... Any clue to save them both ?

First, I'm not sure if high loyalty is relevant here, but it only comes into play if you order them not to go down.

In order to save Kurt, both him and Kasha need to climb down. When Kurt gets ready to cut the rope you need to tell him that he can pull Kasha up. He'll ask if you are sure about that, then you tell him "You are Kurt! You can do it!" then he pulls off some acrobatic move and saves both of them.

i did exactly what you wrote, the only thing different, is at the beginning of the scene, i "let them go" down. I did not "order them to not go".
My loyalty was very high to both of them. I tried the 3 sentences after encouraging him ("You are Kurt you can do it", " Don't let me with Templeton", and the other ones), I also tried to threaten him to cut the rope as it was suggested in the Achievement guide, but the issue is always the same, Kurt suffered from Heartattack and fell off, only Kasha and its camera were saved. I remember, the first time we heard about Kurt's heart trouble, you have a choice either to order him to rest, or continue to drive his scouting team. I did not force him to rest. Maybe this choice had a consequence in this issue...

Many scenes before that, when Kurt asks to speak to you with the Doctor and his heart issue is revealed, do you order him to take it easy, or still let him lead one of the boats?

I've always ordered him to take it easy (or at least ordered him to NOT lead one of the boats) at that time, and he never had a heart attack for me. It's possibly related?

yeah it's confirmed, that's the issue. if you dont' order him to rest, he dies later on from heart attack whatever you try to encourage him tu pull up

you have to order him to rest when you first find out about his heart issues in the doctor's tent!
