There's actually 5 new scars. But I've only found the location of one, which is in the middle of the lake where you pick up one of the heirlooms in the third biome. Can't miss it when heading there. As far as the rest, I think one might be in the tutorial, but I dont know where. And another should be in the last biome at the bottom but I don't how how to get to it. But these are my opinions and I might be wrong.

Mysterious key room is in the tutorial area at the top of the 1st tall building, you can drop down below where the little campsite is. A quick note, the last hidden scar (And some lore books) is in there but only after you have beaten the final 2 bosses in their prime form - which requires NG+7

One of the other new scars can be found in Pishon Dry Lake by going to the Storeroom and into the secret study (found by tapping on the "interact" button while standing on top of some boxes on the right side of the room; there is no entry prompt). From there, you'll find a breakable wall into another small area with the scar. I think that one might require beating Prime Tubal.

There is another new scar that can be found above the gongheads. The roof has a breakable wall which leads into a hidden area where you press interact (no prompt of course) halfway through to enter the secret room.
