It just means your character is gay. It doesn't do anything as being gay is "natural" That's why it's called "Nature"

I know in the first game it changed the gender of the statues, but I did not do anything.

It refers to LGBTQ in general. Comes with different quotes too, like you like both men and women, you are a cool dude/a lovely lady, you are into men, etc.

iirc it does nothing in terms of affecting gameplay, as others have said does affect some dialogue. pretty much just a nod to LGBTQ or something along those lines.
