There's also an addon available on Flightsim.to called RandFailuresFS2020.
So just to clarify. If I am it and set the fail time to 30 then within 30 mins it will fail? Just want to make sure it's a correct assumption.
Another question: when having an engine fire does the game actually show flames or does it just react to the failure?

If both are the same, the failure will occur at the determined time in minutes. If "Fails In" is 0 and "To" is 60, the failure will occur at some point within 60 minutes of starting the flight.
Don't think the game has any visuals relating to failures unless they are programmed in to the aircraft and I don't believe the default aircraft have that level of effort put in to them.
