How does the game handle payware aircraft in multiplayer sessions
Been thinking of buying a plane and there are a few free ones available.
I was curious what happens if you fly one of these in multiplayer where the other players don't have them.
Do they see a stock plane, does it download the exterior mesh, or does it just show nothing?

Any plane bought through MSFS 2020 "store" in game from first or third party will be shown as the specific model selected during multiplayer, for example, player 3 bought a A380, player 1 and 2 sees a player 3 in the A380 taking off.

So if it was bought through the in game store, the external view of the model is available for other players to see.
Specificly I was thinking for the M20R that Carenado just released which is pretty distinctive and doesn't really look much like anything else in the game.

I was flying the Shock Ultra in multiplayer with my brother who only got the middle tier. He saw a generic low wing prop plane.

This is because of your/his settings. In there you can find something similar to "aircraft model used in multiplayer" (it's to save memory or something)

Yes. If it's from the MSFS store then it will show for everybody in a multiplayer game IF they got the settings correct as I mentioned in the quote above. All though I am not 100% sure if it might be a bug on his side or if the settings were not set correctly.
I belive the setting is Options > General > Graphics > Use Generic Plane Models

If you set that to off, I believe that should work. It does for me at least... but again, MSFS2020 works for some and not for others... which is sad.
