Report a translation problem
You have to specifically get 3 perfect medals during a PvP match with, weapons, hard to tell which weapons, and a few that are allowed don't match up with the challenge discription.
To get a Perfect Medal. You need kill a target, where you land every shot and the finisher needs to be a headshot. I'm under the impression you must have caused 100% of the damage, and you can't deal damage from alternate sources like grenades.
Known weapons that can trigger a perfect that are counted for sure are:
Battle Rifle
Shock Rifle
Stalker rifle
Mangler can get it as well I believe, 3 shots will kill with those.
I haven't successfully gotten one with the Pistol yet, but I wouldn't be surprised if that counted.

I did it in BTB, usually because there's a lot of people who mindless rush and places to hide, but also loads of weapons that qualify for the task listed above. Usually loads of Battle Rifles on the map as well.
You could also do ranked matches that start you with Battle Rifles as well, but the Shock Rifle seems to be the easist weapon to do this challenge with, as you only need to headshot the spartan once to get credited. But you'll might be fighting people over that weapon, as there are only normally 1 or 2 of those weapons that spawn on per map.
