To get the "Undetected" award, even with suppressed weapons, as soon as you explode a grenade/flashbang you lose the "Undetected" part unless you can kill everyone whilst they're all still stunned.
At the moment the only guy I can shoot at yet still remain undetected is the guy in the left most car by the left door, as soon as I shoot at anyone else every enemy reacts and I lose the "Undetected" award, even if I still finish the mission with 3 stars.
Yeap, it's definitely a tough one!
It's a bit hard to achieve it...
Her PM63 SMG(found among the 'enemy' weapons) has a sound radius of 20m.
Her Suppressed .45 pistol has a sound radius of only 4m. It also has an incredibly high base crit chance, so it's very likely to drop whatever she can hit with it.
So, strong suggestion: Switch Fergie to her pistol at the very beginning, as it's probably both considerably more lethal and quieter than her primary. Remember that units on 'silent' will still open fire on enemies that are threatening friendlies, so unless you don't let her see anything/leave her crouched, there's always a chance that her not-really-silenced primary could end up giving your presence away.

Remember you can mouseover enemies to see their view cone.
Definitely keep distance from people you're not intending to shoot right away(don't play as if 2.5m or 4m is a definitive 'they can't hear anything at all beyond this' range).
Pay attention to who/what's behind what each of your people are shooting, as you don't want stray rounds to 'wake up' anyone you didn't intend to. Move off to the side if that means keeping other enemies away from the line of fire.
You can force your people to switch targets by moving their view(right click) to look away from their current target while UNPAUSED. It won't work while paused.
Once you're on the bridge it gets tricky, have different soldiers aim at different enemies for a simultaneous take-down.
Last tip, clear the house first before you approach the group all the way to the right behind the jersey barrier, I actually used a grenade on them after the rest of the map was cleared.
This is one of the hardest ones to do stealthy.
Edit: I was able to complete it in singleplayer after we tried for awhile. I still don't understand all of the mechanics behind detection. With my plan, it took a number of runs before I ended up with the right conditions.

I did it but took no video. If I remember correctly, you need to have one guy aim at the enemy that is standing just beside the window, while you use other troop to walk in front of the window and kill the enemy in the back of the room.
When that enemy at the back sees you, your guy will automatically engage, and so will your other troop that was aiming for the first enemy (which does not see you).
After that don't bother killing all the troops, just get the hostage in the house and leave.
