No it isn't!
There are numerous missions where the enemy QRF "spawn" out of thin air either inside or outside with NOTHING around them, no stairs, no "drapes" or any poor other excuse to try to hide bad mission designs around and people making excuses for poor mission design aren't helping the game designers make the game better.
Hell, even the latest update introduced a mission with EXACTLY this issue in one of the new Tiny Troubles missions where and enemy QRF spawn out of thin air in the bottom left corner of the map. My assumption is that the mission designer is trying to simulate the QRF arriving by boat, so just add a boat model (plenty available) rather than having then spawn out of thin air.
Firstly, in what mission do they spawn in a middle of house, as said before in all the official maps all ive seen of them spawning is from entrances or stairs.
Secondly having the enemy spawn is the only current way that a QRF can be implemented. Making a moving boat come and drop enemies off is just not possible at the moment, also having enemies run onto a map from the out of bounds is also currently impossible(A map maker could answer that a bit better than me). These QRFs also generally appear from areas that are quite obvious. roads, side gates exct. If you see it it might be jarring to see them pop out of the air but it doesn't ruin the map suddenly
Door Kickers 2's control enhancements over 1 make maintaining security a lot simpler and easier to set up.
