I dont get how you've managed to make it so you only press the go code once. I press it and they walk into the wall unless I press it again to blow up the breach charge.
If I am correct you:
1. Placed the charge with orange and then assigned the blow up order to go code A.
2. Walked orange away from the charge a bit then drew the path into the room for orange and then assigned that path to go code A too.
3. Drew cyan's path into the room and then assigned that to go code A as well.
4. Signalled go code A?

So, seems that it's only if the charge is placed on doors that a double command is needed...

Use a different gocode for them and it should work.
This is all because you can reuse gocodes along the path. In theory you can only use A for a whole plan: the game will only execute the A you reached and leave the others alone. When you reach the second A you can execute that one and so on.
But if I place the charge against a wall, set go code A for the charge and go code A for the soldiers which are some meters away from the wall, I can both the actions with a go code only. The difference is with the charge planted on a door...
detonate with A and move with B, as Pintea wrote.
Pressing A and B is lazy, but alternatively 1 and 2 is easy.
This game has "which object to contact" issue, a.k.a hostage breaching,
so complicated order would be some kind of debugging...
