Most of the time the hobo's started it, ichi is just defending himself. Or maybe its the schizophrenia idk

I'd say undiagnosed schizopheria. basically he needs medication, and due to his fixiation on dragon quest in his youth he envisions his enemies as game-styled caricatures.

This is just a guess though. He might just suffer from maladaptive daydreaming. Japanese prisons are not known for their healthcare, or mental illness counseling.
