I'd say undiagnosed schizopheria. basically he needs medication, and due to his fixiation on dragon quest in his youth he envisions his enemies as game-styled caricatures.

This is just a guess though. He might just suffer from maladaptive daydreaming. Japanese prisons are not known for their healthcare, or mental illness counseling.
