As far as I can tell, some event choices are circled with a white/black serpent IF you have experienced them before.
Also, after killing Sigismund I think you fight Milenis in white and Mortis in black
The bar and two snakes on the top of the screen in the karma system. White snake means 'good' actions and black snake means 'bad' actions. This system helps you to get your individual game ending - good or bad. Before you choose some options in events you can see the same snakes (black or white) on them, this means that this action will affect karma.
Millenis awaits you if your karma is good.
