Dude, you're elite. Not casual.
I actually do that to some extend (eg. I avoid xp boosts from drops, I use teleport only in rare cases, etc.)
I know you probably don't like that answer but there is no such server you ask (at least that I know of).
I can tell you though, the game suddenly becomes way more interesting playing that way ;)
That only if he's planning on playing only PVE, because not using that stuff will put him at a disadvantage in PVP
there is one server with absolutely no cash shops and OP freebies. AO Reborn.
I haven't check out it much, but as far as cosmetic item goes, they are easily accessible and no stat differences.
I will actually encourage people to support private server rather than keep throwing money at Valofe.
1-81 = free trial.
81-170 = tutorial (weening off main quests onto dailies and dungeons, later avalon etc.)
171-200 The real game and BY FAR the biggest amount of time spent here.

thanks for the AOreborn mention. Currently downloading it as we speak to test it out .
Some things to note:
-AO Reborn is a much earlier version of the game. ie. no warehouse, inventory is somewhat limited, many 'quality of life' things that are in the current AO version are not there, etc.
-Blademaster is supposed to be enabled but it really does not work that well (if at all imo)
-some bugs here and there
BUT
-all licenses are permanent, including teleport, all merc rooms, all extra inventory bags, patrols, auto-battles,etc.
-all characters start with a mount that has a magic ability
-items (quest or not) have a way higher chance to drop
-guild crafts start at +3
and more things that I still haven't seen (still low level char)
Most important though: NO Item Mall, server owner always responds through Discord and no immature players. Everyone I have met so far was well-mannered, polite and they had a general air of 'maturity'.
