A Quick Alchemy Lesson
The player has five attributes.

Endurance + Wisdom + Power + Agility + Luck

Each attribute has a corresponding vegetable or plant that you grow in your garden that you will use as your potion BASE.

Endurance - Alluring Fruit
Wisdom - Mana Nectar
Power - Chili Pepper
Agility - Strawberry
Luck - Goldenberry

Each potion has a color that corresponds to a gem of the same color.

Endurance Green potion Emerald
Wisdom Blue potion Sapphire
Power Red potion Ruby
Agility Yellow potion Topaz
Luck White potion Diamond

Access the Alchemy Set in the morning or afternoon at Lily's shop.

When the screen opens, you see four slots. The first one, larger than the others to remind you, is ALWAYS AND ONLY for a potion base (that is, as above, things that you get from your garden)! The other three are for ingredients.

The other thing you see (along the bottom) are recipes that you know typically obtained via books that you buy from Lily or from quest givers (because they want something you can make).

It is important to know that not everything you make is a potion. Typical things in the game that don't need a base are Green Tea and the special rope for Ginny and all gems. All gems require 3 fragments, for example, but always use just the ingredient slots to make them.

Ingredients are best found in your garden, by fishing, by going on Keros's mini-game and by finding things in the dungeon or even buying at least one thing I can think of from a vendor there.

When experimenting to see what's possible note that ingredients have power ratings for a reason. Hint. :)

To finish up and so you can see how this all comes together, here are two ways to make a +3 potion for any attribute. Each + that a potion has adds 5 to that attribute, so +3 blue means +15 Wisdom.

BASE + GEM (of the correct color) Example: Mana Nectar + Sapphire
BASE + STURGEON + STURGEON

This method is not the only way to do things. You can also make a blue +3 from 3 ectoplasm in the ingredient slots. It is intended that the player experiment. Have fun!
