Report a translation problem
If, like me, you're using a wireless controller, try plugging it in with a cable, or use M+KB for a while.
Incidentally, removing my USB BT receiver, rebooting the PC and plugging it back in seems to have resolved the issue yesterday. I'd say it's still the game at fault, but there might be some bad interaction with BT and Windows (I remember Win being awful with BT some years ago).
After sorting that out, the game's been running perfectly, v-synced to my screen's 75Hz on Very High.
