Witch Cauldron and Mysterious Forge Recipes
I'm starting a thread that everyone can contribute to in order to get a list of all the possible recipes. I have had these so far:

Mithril Axe + Mithril Sword + Mithril Dagger + Axe of Diastrophism = Amulet of Pisces
Tower Shield x4 = 14 Middling Essence
1 Waterdrop Topaz + 1 Waterdrop Emerald + 1 Waterdrop Amethyst + 1 Waterdrop Sapphire = 2 Tears of A Star (confirmed every time the same)
2 Pentagonal Topaz + 2 Pentagonal Sapphires = 1 Powder of Haste + 1 Angel Statue
Cosmic Essence + Mysterious Essence + Greater Essence + Middling Essence = 1 Dragon Blood + 1 Potion of Fate
Waterdrop Emerald + Waterdrop Sapphire + Waterdrop Topaz + Self Portrait of A Writer = 2 Tears of A Star (this leads me to believe 4 of any orange material will produce 2 Tears of A Star, but I haven't tested enough yet.
Mithril Bow + Mithril Dagger + Mithril Staff + Mithril Hammer = Adamantite Sword

Things are a bit more value and rarity based than specific items for the forge, for the Cauldron it might vary a bit more. Might add a section on it in the guide but I think some recipes have slightly changed in the recent update so I've been holding off on it.

I've been experimenting a little bit on the forge and cauldron today, what I found is there isn't really any specific recipes. I found the best way to get good results is by a varied mix of items ( just putting in multiples of the EXACT same item seems to result in failure ) and having a few decent items in the mix.
So it seems like it might be based on quality and variety, which then creates a random item. There's probably a formula for optimal results, personally for the cauldron I was using a combination of unwanted decent quality items and then filling up the pot with chaff.
For the forge I was putting in 5 purple varied items and getting a random red item from the next tier, it's possible though that I perhaps didn't need as many as 5 items or that I could have filled up the pot with a few items of lesser quality to make up the numbers.

Aren't they just RNG? If you are strong enough to defeat post game boss, you can just farm her to collect your gears.

I know 5 epic weapon/gear will give 1 legendary random gear. The result seems random regardless of which part you put in.

Nice Forge guide.
For cauldron I've been using
1 red
1 purple
3 blue
2 green
2 white
This gets me 2 Tears of a Star.

A couple of times now I've had 3 Tears of a Star with these items, I'm not sure if it was random or a result of the kind of items I was putting in. I believe the times when I got 3, the blues and possibly the red I'd put in were gems, and the whites were stacks of ore/cloth ect. that I'd put in just to get rid of.
As before though, all 9 items/stacks were different.
ie. if I put in a stack of 6 mithril ore then that's counted as 1 item.
I'm not sure if it was just random, or if there really is a more optimal mix down to types of blues/red and/or the white objects being two small stacks.

I always just used two or three ingredients for tears, back then it was guaranteed a stack of three but I think during one of the updates it changed from 3 to a chance of 1-3. I'll see if I can make a guide on that, I like to reconfirm every single recipe just in case so might take awhile.

I dont think there are any recipes, just insert 4 different items of same grade will yield one random item of higher grade

The recipe guide, so far, is inconsistent and very random
In my case, 4x purple will most likely give me spirits instead of items

For the forge? Purple x4? I've never once had a different result, wouldn't match the formula either unless they added a new chance modifier or possibly linked to difficulty setting or game progress? Even still it would most likely give at least an epic or rare, if you're getting essence it means it failed which makes little sense. What difficulty are you on and have you reached end-game? Might be possible you're experiencing a bug.

Edit: Wait, I think I know the problem, are you trying to craft 4 purples of the same item? That will definitely give you essence as stated in the guide. Stacks do not count, each purple has to be a different item.

Yes, it explains much now.
I still have an occasional spirit result, perhaps accessories can't be mixed with weapons and armors. Will verify in future encounters.

It looks like the latest patch has altered the cauldron and forge, it seems like the results aren't as good as before for the same material cost.

It is mostly depend on your ITEMS FOUND stat.
I hack that stat to 1000% and I got 5 gold items (each one is different) from 9 red items.
Normally it will give you 1 gold item.
