how to unlocked the threesome feature

What you need to do is do one of the sisters and the other comes in on it. Afterwords for the Foursome you need to be doing the threesome and the Aunt comes in on it.

Darkness is right, but to add:
100 VL will cause people to walk in on you.
Usually it is 100>X>=80 VL you'll get a convo w/ them + a bomb icon (think Uncle skips this step).
Once you hit 100 VL, the person will walk in on you.
Boku needs to have had sex w/ the person walking in on you too, or game over.
If the sisters catch you w/ mom first, game over.

80 VL Sisters > 3-some sisters > 80 VL Aunt > 4-some Oyakodon.
