Do it with one of the cousins, then raise the alarm level of the other cousin to the max. If you have max affection with her, then you unlock the threesome ability. Then raise Miyuki's affection to max, have a threesome, raise her alarm level to max, unlock foursome. Just watch out not to get noticed by the uncle.
