Early levels were hard, and my warriors would need sword and shield to survive (2 handed weapons didn't work). After level 10 (roughly), and everything is straightforward.
2x warriors - One is 2h and one is sword and shield - Both wearing heavy armour.
2x mages - AOE everything - Both wearing light armour.
I suggest working on levels and getting some of the higher tier skills. Your warriors should eventually be able to aggro all mobs and survive through shields + force fields. This will allow your damage dealers to wear light armour. Sometimes you may need to go to tactical view to move the ranged characters away from enemies.
This is my first playthrough, so don't take my words as a guide - Just my experience so far. Currently level 15 and I've not faced any difficulties for the last 15 or so hours.
Two handed warriors are pish poor unless you are playing as one, you shouldn't take anything out of the vanguard tree either except charging bull
The only Rogue that's good in dual wield is Sera, Cole and Varric should remain ranged
If you're using two mages that both use barrier you might need to change tactics because they'll usually both cast it at the same time effectively wasting a cast
Set your sword and board tank to prefer war cry and payback strike with the upgrade to taunt enemies, and shield wall, this way they'll maintain aggro while being defensive
I only ever use their default armour class, only use one warrior, play on nightmare with trials and never have any issue's, there's only a couple of battles in the whole game where I'll take control of another party member, otherwise I usually play for like 98% of the game using the player character
