Relationship Levels

Hostile: -75 to -30
Cold: -29 to -5
Neutral: -4 to 34
Warm: 35 to 74
Friendly: 75 to 125

As a companion's approval increases (or decreases), their feelings towards the Inquisitor will also change. Initially, they are at "Neutral" upon joining. As the approval changes, their greetings and dialogue will change slightly to reflect their relationship.

It can be gauged by examining the companion's card on the Gather Party screen or by the changes in their greetings.

The approval rating is somewhat tied to the Romance options and there's no gift system, there's no bar to tell you how high they approve or disapprove of what you do. For all of the Companions you can get their quests just through the natural progression of the game and at least having them approve mostly of what you do.

If someone's approval rating is low enough they will actually leave. This is confirmed to be the case with Blackwall, Dorian and Cole. It does not happen with very story critical characters (Cassandra, Solas etc) but they will hate you and you will get an interesting cutscene (punching Solas, Cassandra getting drunk and telling you off).

When you become high enough approval and friends the card art changes.

approval is affected by dialogue choices or also the actions from some quests and story made choices In game from main quests.

It is totally possible if you're careful with who you take on quests and choosing the max approval dialogue options when chatting with each companion. For major decision points and judgements, you can't make everyone happy, but usually there will be some way to make up for it. In one of my playthroughs I maxed out the approvals with Cassandra, Solas, Varric, Blackwall, Sera, Cole, and Iron Bull (they no longer gain approval when I do something that's supposed to) despite these people having very different opinions on things. Most of them have fetch quests and I make sure I take them on these quests to gain approval. Dorian and Vivienne have fewer fetch quests for approval than others, but I still managed to keep them at 75+ approval. Iron Bull is quite easy. Just take him dragon hunting. Cole likes it when you're helping people even when he's not in the party. I saved all my Hinterlands refugee quests until he joined me and then I completed them all at once. I got his Assassin's Hat within an hour or so.

Dragon Age Wiki's approval guide is very helpful. The wiki has approval guide for each side quest as well. Mind you though, farming approval feels more like a chore and it's not really a lot of fun.

One companion leaves after the final boss battle no matter what your relational state with them is.

One companion is unavailable for the final boss battle if you fail to do his loyalty mission.

One companion actually does turn on you in the DLC if you didn't play your cards right in their loyalty mission.

Sorry if this is vague but any more info and I'd spoil things.

BTW important safety tip: anyone who leaves, either of their own volition or when you sack them, takes all their gear with them. Verb. Sap.! :-)

Spoilers kinda:

They leave because they are part of the Dlc story trespasser.

The companion that is unavailable makes sense when you find out who it is and what they are but you are able to use them if you complete Thier personal quest and are available after the fight too

Again the one that turns on you makes a lot of sense given the decision and lore surrounding them and what they are.

You don't have to farm.. you can be friends with quite a few just rping just not all of them given how they have conflicting views on issues.

Also the betrayal thing is only for one companion and will always happen unless you stop it in their personal quest.. due to lore surrounding them and what they are.
