Usually, an exception gets thrown because of a buggy mod causing unexpected behavior and the game responds by halting the action and doing a "Reset this Sim" on the Sim that caused it to be thrown. (Which you can often see when they suddenly pop into a T-pose for a second.)
Keep in mind, sometimes exceptions get thrown for no good reason. And sometimes due to bugs and weirdness in the official files (Crystal Creation has a bug that'll do it, among others) and there's no real fix unless EA fixes it. AND, sometimes even after an official fix it'll persist in the save file, but won't happen on new saves. (Which is actually fairly common with video games in general.)
