Repair your game:
Origin-only version:
  • Go to your library
  • Right mouse button click on The Sims 4
  • Click Repair
Steam version:
  • Go to your Steam library
  • Right mouse button click on The Sims 4
  • Select Properties
  • Go to Local files tab
  • Select Verify intergrity of game files

Remove localthumbcache.package located in %UserProfile%\Documents\Electronic Arts\The Sims 4\
If you are using Mods/CC, please make sure:
- major mods are up-to-date (compatible with latest game version)
- you don't have any conflicts with CC
- you have disabled "situation" and "allow for random" flags for specific CC (usually these flags cause problems with random-generated sims and outfits).

I have tested all that.

I had this issue as well not to long ago and I also did the repair and the verification, but it did not help the issue. The only way I was able to resolve it was to load the previous save and everything was back. Then I deleted the save that was messed up. It was like the save I was using didn't fully save before the game closed.
