There are many ways to make the game more realistic with the packs, Get To Work, for example, adds an illness system, you can have babies at the hospital instead of going into a rabbit hole and really cool careers, the doctor career is my personal favourite. packs like High School Years and Discover University, make education more in-depth, tho High school years gets boring after a couple of days, So Discover University might be better for that. Then we have realism for families, Growing Together, Parenthood and the upcoming Lovestruck pack (tho we're yet to see how good that pack is but so far it looks amazing) are good for family game-play, AND I CANNOT FORGET SEASONS more plants, adds weather and holidays, Seasons is 100% a must-have. Hope this helps :)

technically you dont need any, however as to which pack you will enjoy the most is impossible for anyone to tell each simmer is different we all play the game differently we all like different aspects of the game a pack i love someone else might hate, all i can really recommend is checking out some reviewers like LGR watch his videos on each pack and decide if that pack seems like something you are interesting in having (and of course unless you have deep pockets always wait for a sale if you can)

Note even though someone on Steam would be fine with NSFW content, loverslab is know for having tons of porn mods. So if you're sensitive to that stuff you shouldn't go there (yes I know you need to be 13+ to play the sims 4 and be on steam)

The DLC you need for a realistic game in my opinion would be Seasons, Cats & Dogs, Get to Work, and Get Together, and Growing Together (for expansion packs).

Earliest expansion packs (Work, City, Get Together) and some early game packs (Vampires, Parents thing)

Seasons have not much content and city is worse than the earliest game packs

Lovestruck does not look like a family gameplay expansion pack. It is probably focused on only just young adult and older mostly. Not even teens are likely to be included for this expansion pack besides CaS (clothes, hair, tattoos, traits, and etc...). However, it is probably not going to be one of the first expansion packs that someone should buy or at least not until it has been out for at least one year since release date. Also, HSY is not as bad as it was when it first came out but its Steam reviews have not went up that much since it first came out almost two years ago and probably not one of the first expansion packs someone should buy similar to Lovestruck, Discover University (build mode is good and especially CaS that also adds a new teaching career but the university experience is kind of disappointing and not really all that interesting in my personal opinion), Eco Lifestyle, Horse Ranch, and For Rent.
