(1) Quit the game.
(2) UNZIP the mod. This should result in a folder once unzipped. Take that whole folder and copy or paste it into the Mods folder. MAKE SURE that the files inside it are ONLY IN THAT FOLDER, and aren't in yet *another* sub-folder within the one you just placed in Mods. There will be a .script file and probably a couple of .package files. All the files for that mod can be left within that single folder.
(3) Start the game. Don't load a save or start a new game. Go immediately to Game Options and select "Other". Go down to "Enable Mods" and "Scripts". Make sure those boxes are ticked. Click "Apply changes".
(4) Don't start the save or a new game yet. After applying the Settings changes, QUIT the game again. Now, restart the game. If you followed all instructions, the mods should be enabled.
As a side note, you may also be referring to "Custom Content" - which is different than "mods". If you're downloading a Sim, then you'll need the Tray files. Place those in "Documents > Electronic Arts > The Sims 4 > Tray." Custom Content for a Sim goes into the Mods folder. I also suggest using 7zip.
Create another folder somewhere else, name it like Sims4modsbackup or something like that, there you put your downloaded files.
Get 7zip to unzip files and store them in said folder to have a backup.
Now go back open the Mods folder and copy your mods:
Let's say you have a mod RabbitGirlsPinkBunnySlippers.zip, you unzip it with 7zip to your Sims4modsbackup>RabbitGirlsPinkBunnySlippers, then open that folder, there is something like PinkBunnySlippers.package.
Now you have to check options in the main menu of Sims4, start it and find the options enabling mods and checkmark them.
After exiting the game you either copy the whole RabbitGirlsPinkBunnySlippers and check if it's listed in the mods list popping up in the main menu.
If not you go back, delete the folder and copy the *.package file. Check again if it works now.
