I know this can be a lot of work, but the only way to prevent this from happening randomly is going through as many sim households as possible to make them straight in the sexual orientation part of it. As for pronouns and transgender sims do not worry about it. Very few sims including even from dlc are by default nonbinary/transgender and only other exception are some of them being shared from gallery which you could also chose to ignore rather than add to your game.

the + and - define the him her thing the same with computing or math there can only be + or - not -+ of course im talking biologically if someone wants -+ it = x but thats not biological that is disire of personality or ego to be whatever pronoun it wants to be
I do not have a problem with pronouns on The SIms 4. They/Them (or she/they or he/they which are similar to they/them) I think is the bigger controversy behind pronouns than he/him and she/her. Also, most of the sims by default (such as Landgraabs, Pancakes, Goths, Zest, randomly generated sims from neighborhood stories, and etc..) besides those shared from gallery do not use pronouns. The only controversy behind he/him and she/her is if a sim is a female but has male pronouns or a sim is male but has female pronouns, but pronouns are not randomly generated by default.
