there should be two nurses that will tend to patients and clean up after a patient has been treated and left.
a receptionist to welcome sims to the clinic and send out an alert if there is a really unwell sim or a pregnancy about to happen.
unfortunately the nurses tend to talk to patients a little too much and block access to the correct side of the bed, so your sim can not gain access to the patient until the nurse moves off. the only none cheaty or mod way around that is to move your sim into a nearby hallway and call the nurse over and then go and treat the patient. if the nurse has no neat trait, they are unlikely to clean up by themselves.
another issue is how long it takes to diagnose a health issue with a sim as sims will leave the clinic after a few hours if you haven't helped them. there are also sims that have no health concerns at all that mix into the rest and waste your sims time.
you got to prioritise accordingly and try get the best diagnosis as possible to cure the unwell sims and find out those wasting your time so you can deal with those who need the help.
you need more homeless sims that the game can assign for the roles in question, otherwise it will stay like that. either create homeless sims yourself or consider a mod like MCCC that can create homeless sims in the world.
